Patient intake company asked if they could sell my health information

I was filling out some preappointment forms for my doctor's office today when this popped up. I'm not the best at reading these things, but as far as I can tell, they are asking if they can use my health information, not just that I submitted to them today, but also all health information my doctor has, to advertise to me. What the fuck.

And most people are just going to sign this because they assume it's a normal part of filling out the forms, it's going to be hard for people to understand this is separate and not required of them.
